Enrollment Requirements
All Adult Ed Programs

Complete an application and pay the non-refundable registration fee to reserve a seat
- Submit a certified high school transcript or GED (for all Medical Courses and anyone who is applying for Federal Student Loans and/or Grants)
- Submit Background Clearances
- Have funding sources secured and submit any award letters 10 days prior to starting
- Must be at least 18 years of age
- Have dependable transportation to and from class and clinical sites
Practical Nursing Program Enrollment Requirements
Complete the TEAS examination (TEAS scores will be accepted for 5 years)
CDL Class A & B Training Programs
- Pass a DOT Physical & Drug Test prior to the start of class (Must be done at LVHN Schuylkill OccMed)
- Have a valid driver’s license
- Have an acceptable driving record (MVR)
- Federal Motor Carriers Fleet Safety Compliance Manual Part 391.11 (section) (a) (2) indicates that the driver shall be able to read and speak the ENGLISH language sufficiently to understand highway traffic signs and signals and directions given in ENGLISH and respond to official inquires.

Student Right-to-Know and Campus Security Act, passed by Congress in 1990, requires institutions eligible for Title IV funding, under the Higher Education Act of 1965, to provide information freely to the public especially for current and prospective students regarding (1) “completion or graduation rates of certificate- or degree-seeking, full-time students entering that institution”, (2) “graduation/completion rates of all students as well as students receiving athletically-related student aid by race/ethnicity and gender and by sport, and the average completion or graduation rate for the four most recent years”, and (3) institution’s crime statistics.
